The deramp code in pycuampcor estimates the phase gradient phaseDiffX assuming X is the slow axis but then applies it assuming X is the fast axis. This PR uses a consistent indexing.

This is a WIP. I'd like to add a way to inspect the estimated gradients so they can be compared with a simpler implementation in a unit test.
