Enhance Vision mode to support multiple file uploads - #3316

A blocked file chooser hands the next steps to upload_file, and each file takes its own handoff. A run could hand off at most twice, and handoffs whose upload worked counted too. The third asset of a GitHub release was refused with "file uploads are not available in vision mode", and the agent asked the user to turn Vision mode off right after two uploads had worked in vision mode. Now only handoffs that end without an upload (expired, or the page was left) count toward UNSUCCESSFUL_UPLOAD_HANDOFFS_PER_RUN (2). Past that cap the chooser is reported with its own reason, exhausted: the earlier attempts attached nothing, so the user can attach the file by hand or run with Vision mode off. Another chooser in the batch that armed a handoff is told it is handed off too, instead of getting the generic vision-mode text. The handoff guidance now says to click the upload control again for another file. A refused handoff leaves a vision_upload_handoff trace note (stage refused; reason exhausted, unreachable or no_source), which metadata traces keep. Tests cover both builds: three files attached in one run, two handoffs that attach nothing followed by the exhausted message, the counter, the batch case, the trace notes and their projection.
