Add inequalities, facilitate HomQCQP creation

Three main improvements

• Can now add inequalities to HomQCQP
• Created an easy function to create HomQCQP objects from matrices directly
• When clique_data is explicitly given to clique_decomposition, then the variables will be ordered as expected (in the order they appear in clique_data)
