docs(knowie): sync vision + experience for Phase 32 realtime#88

knowie-judge flagged vision.md as stale after realtime shipped. Fix:
- vision: 現狀 / roadmap intro / bottom status 階段 1–31·rev 93 → 1–32·rev 95;
remove realtime from the descope list; resolve the internal contradiction
(roadmap said realtime "很可能 descope" while it's live).
- vision: add the 階段 32 roadmap entry (/v1/realtime WS relay — capability-axis
detection, per-second/minute metering, in-flight revocation, Azure URL gotcha).
- vision architecture: note /v1/realtime is a deliberate exception to "litellm via
single adapter" (direct websockets, not litellm; first-citizen build-vs-adopt).
- experience: distil 2 lessons from the realtime build —
(1) realtime capability lives in supported_endpoints, NOT a litellm mode;
(2) probe a live WS protocol with real creds + surface the provider's reject body
(how the Azure deployment=/intent=transcription bug was located).
