Migrates the runtime attachment shape onto the canonical schema field names (target_node, order_in_stage) and normalizes legacy persisted rows on read, so the UI stops producing attachments that overlay/compose.ts silently drops. Pure data-model migration — no user-facing behavior change, no new UI. Adds normalizeAttachment + a compose.ts shape-guard test. Phase 0 of the attachments authoring work (#67/#62).
