Integrate device/program agreement form for digital detroit project

• replace TemplateView with a form view
• require login and check signup for view
• populate known data from signup data
• store form data (maybe in signup question JSON in Application)
• update learner dash component to show prompt if the form hasn't been completed and check mark if it has
• update hardcoded form to use django input fields or change to complete django form
• prevent submitting form if you don't check the agree box(es)
• validate phone numbers
• only show program agreement form to digital detroit project learning circles that are running during project timeframe
• use phone number input
• use geolocation input
