Updates the CIEL concept for "Immunization History" (1421) to include a recent child addition, 161011 "Free Text Comment". This initially required removing 1421 (removing the reference only) and then re-adding it to the Head. Interesting Note: this includes a significant increase in CIEL's mappings as well, taking the total # of mappings in this Immunization collection from 113 to 239. A good reminder that we don't seem to be automatically benefiting automagically from CIEL updates unless we do a version update.

Process Note: This initially required removing 1421 (removing the reference only) and then re-adding it to the Head. I think the reason we didn't get the updates to 1421 automatically in the Head was because, even though we were using the correct reference to 1421, 1421 was added before CIEL supported dynamic references.
