Currently clicking doesn't do anything, at least not for me. I expected it to, especially with the hover working and clicking making sounds and an tap animation. Could there be added a setting that enables clicks to go through? Is that even a good idea? I imagine this app is not designed to click much, but though maybe a little could help.
It introduces an explicit Pointer mode that passes native clicks, double-clicks, right-clicks, modifiers, and drags through to macOS, while Draw mode retains presentation-only drawing. The overlay also stops intercepting Dock clicks in Pointer mode.
The PR includes screenshot/video evidence and is open, not merged.

Thanks @silvenon you can hold the option key to click on things. There is a setting for that.
