Norma.Utils.form_url/1 concatenated the #fragment before the ?query, producing invalid URLs like /Path#Frag?Q=Foo. The fragment must come last per RFC 3986. Update the corresponding test accordingly.

Hi, this cleans up and updates the code to run on Elixir 1.19, fixes some warnings and a bug related to fragments.
