Mint a per-runtime instance id so assistant segment itemIds stay unique when Cursor reuses the same ACP sessionId after session/load. Without this, the orchestrator collapses new assistant output onto the prior turn's message. Co-authored-by: Cursor <cursoragent@cursor.com>
