chore: add security gates and smoke tests

Summary

• add Biome, commitlint, publint, Dependabot, CodeQL, CODEOWNERS, security policy, and protected npm release docs
• add manual npm publish workflow guarded by main branch, prior CI success, protected environment, and npm provenance
• add a production smoke test for the built CLI and MCP server before publish
