feat: add runtime intelligence reviewer focus

Summary

• add a Reviewer Focus quick-scan table before detailed Runtime Intelligence risk rows
• render the focus table in Markdown and HTML reports without changing JSON/report contracts
• gate the new focus markers in the Runtime Intelligence artifact bundle check
• document the reviewer-facing quick-scan table in README and CI artifact docs
