I don't think this is a good idea or at least not this UI.

  1. Users won't see the existing issues with a similar title (what Github suggests under the title since a while)
  2. It also doesn't ask whether people have searched for duplicate issues. (I've explicitly added that question for a reason in the template, because even the suggestions from github, probably only on title, is quite limited).
  3. There is no way to add screenshots / screen recordings, which are frequently essential in figuring out what's going on. Being able to add images / recordings has always been one of Github's issue queue great benefits compared to other issue queues.

The only benefit I see is that it's easier to include OS info.

Please see if you can include something that makes it easier for people to discover duplicate issues or reminds them to do some research and enables an easy way to include screenshots & screenrecordings.
