What does this PR do?
Adds GPT-5.4 support across OpenCode provider resolution paths even when bundled or fixture model catalogs are stale. It backfills GPT-5.4 metadata for OpenAI and OpenCode providers, updates Codex OAuth allowlisting, and extends reasoning-effort handling/tests so GPT-5.4 gets the expected variants.
