Refresh docs to better reflect the project state.
PLAN.mdis renamed but kept intact for historical purposes, but it's replaced withDESIGN_RATIONALE.md. Added a preliminarydocs/consumer-transition.mdto document the requirements for consumer transition (only canonical cosmos chains with x/staking and x/slashing are considered for now)
