docs: close p04-p12 remediation review log

• append the Wave 3 closure resolution log to the pinned 2026-07-06 P04-P12 target-state review record
• map F1-F12 to the landed remediation PRs test: restore branch-coverage headroom above the gate floor #72-test: capture EVRUN-full smoke evidence #79 while keeping F3 only partially closed
• keep remaining P11 no-phone-home/repeated-effect idempotency and P13/P14 posture explicitly open, matching current docs
