Adds agent instructions capturing the 3.0.0 release postmortem: version-surface checklist (all files that must agree, incl. src/version.ts SDK_VERSION which shipped stale), the npm provenance case-exact repository.url trap (422), the npm auth requirement for publish (404 without token/Trusted Publisher), the passkey/OTP CLI limitation, and the audit-first-then-publish sequence.
