fix(detector): detect two-slot state replay

KernelGuard-Red-Submission: 272

Adds a narrow LAST_CALL_REPLAY pass for multiple input-derived first-call state slots that are combined in the return value.

The detector requires:

• at least two input-independent guards over state slots
• each guarded slot assigned from a live input-derived expression
• a later return expression that references at least two of those guarded state slots
