The contribution section in the readme was linking to Github releases, which haven't been used in some time. I've updated this to link to the Tentacle download page, where you can also access change history.
The PR template also caused me some confusion, linking to the global Octopus issues repo when Tentacle relies on it's own issues for release notes.
And finally, we made a decision a while back to remove the placeholder images in server PRs - so doing the same here.
