Audit log correctness in transactions and error handling
Rationale
Three maintainer-accepted fixes relate to audit log integrity: using in-transaction state instead of stale pre-transaction snapshots, not silently swallowing audit write failures, and validating enum values before writing audit counts. The one rejected comment was about migration index strategy, a separate concern.
