Add HEARTLAND Protocol FHIR Implementation Guide (resubmission of #592)

Summary

Resubmission of #592 (approved by @grahamegrieve 2026-05-16 but closed before merge due to validate workflow failures).

This new PR rebases against current master and fixes the validate errors that affected our entry:

• authority shortened from "HEARTLAND Protocol (independent peer-reviewed open project)" (59 chars) to "HEARTLAND Protocol" (18 chars), now compliant with the 40-char registry rule.
• country already "us" (lowercase) — was correct in Add HEARTLAND Protocol FHIR Implementation Guide #592 too. The validate failures for "US" belonged to the DAPL/DARTS upstream entries, which @grahamegrieve has since corrected in master (d8dd632).
