The bearerTokenEnvVar and envHttpHeaders fields called out in the side note here were undocumented, so I've added a section to docs/developing/configuration.md (under Plugins MCP and LSP) covering:

what each HTTP-transport field does (url, httpHeaders, bearerTokenEnvVar, envHttpHeaders)
that they hold env-var names, not secret values
header precedence when more than one sets the same header
the exact requires env var '…' for header '…' startup errors and what triggers them
