Root cause: all reviewer candidates on single github-models provider; shared rate limit exhausts -> selected_model=none, blocking approvals org-wide. Adds OpenAI direct provider (secrets.OPENAI_API_KEY) gpt-5-mini/o4-mini reasoning=high as fallback before rate-starved flagships. Contract test updated. Generated with Claude Code
