Summary

  • Scope OpenCode coverage evidence checks to changed source files instead of all repository source files
  • Keep backward compatibility for existing no-source coverage evidence phrases
  • Update quick-gate fixtures and assertions for the changed-source coverage path

Validation

  • py -3 -m py_compile scripts/ci/opencode_review_normalize_output.py
  • git diff --check

Note: full shell self-test is unreliable in this Windows checkout because WSL/Git Bash hit CRLF and Python shim issues before reaching the changed logic.

OpenCode Review Overview

  • Head SHA: 430e57cfb1cf512063b45874493c02908d2eb47d
  • Workflow run: 28518778592
  • Workflow attempt: 3
  • Gate result: REQUEST_CHANGES (approval step)

Pull request overview

OpenCode exhausted the configured model pool without a usable current-head review conclusion. This is not approval evidence, so the PR is blocked until a source-backed review can establish approval sufficiency or identify concrete fixes.

Findings

1. HIGH .github/workflows/opencode-review.yml:1 - OpenCode could not establish approval sufficiency

  • Problem: every configured model path failed to produce a usable current-head control block.
  • Root cause: model execution, timeout, export, normalization, or approval-gate validation did not complete after exponential retry across the configured model pool.
  • Impact: approving from deterministic check state alone would miss PR-intent mismatches, missing files, edge-case bugs, robustness gaps, UX/DX regressions, security issues, and CodeGraph-backed base/head flow changes.
  • Fix: rerun OpenCode after model availability recovers, or update the PR with the missing files, tests, docs, generated artifacts, and verification evidence needed for a source-backed review conclusion.
  • Regression test: keep the approval gate posting REQUEST_CHANGES, not APPROVE or check-only failure, when no model produces a valid current-head review.

Summary

  • Result: REQUEST_CHANGES
  • Reason: coverage-evidence passed and peer GitHub Checks completed without failures, but no model produced a valid review control block.
  • Deterministic evidence checked but not used for approval: current-head changed-file evidence (.github/workflows/opencode-review.yml, scripts/ci/test_strix_quick_gate.sh); coverage-evidence result success; peer checks from statusCheckRollup excluding this OpenCode check.
  • Model outcome: model_pool=exhausted; selected_model=none.
  • Head SHA: 430e57cfb1cf512063b45874493c02908d2eb47d
  • Workflow run: 28518778592
  • Workflow attempt: 3

No PR approval was posted because model-output failure is not evidence that the PR has no blockers.

Pull request overview

OpenCode reviewed the current-head bounded evidence and found source-backed failed-check findings that must be addressed before merge.

  • Result: REQUEST_CHANGES
  • Reason: failed current-head checks were mapped to line-specific findings below for d7b9c4869bfd7f44a1e07179dc33bf02832d88b3.
  • Head SHA: d7b9c4869bfd7f44a1e07179dc33bf02832d88b3

Failed checks

Findings

1. MEDIUM GitHub Checks queue - PR Review Merge Scheduler/scan-pr-queue was cancelled by a newer queued request

  • Problem: PR Review Merge Scheduler/scan-pr-queue did not produce reviewable source evidence; GitHub reported - .github:1-1 [failure] Canceling since a higher priority waiting request for central-pr-review-merge-scheduler-ContextualWisdomLab/.github exists.
  • Root cause: GitHub Actions cancelled an older queued or running check because a higher-priority request for the same PR was waiting. This is a check orchestration state, not a source-code defect.
  • Fix: Do not approve from this cancelled context and do not paste only the workflow URL. Wait for the newest same-head check run, or rerun the check after the queue settles, then review its actual logs.
  • Regression test: Keep failed-check fallback reviews explaining cancelled check contexts separately from source-code findings so cancelled jobs cannot hide an actionable pytest or Strix failure.
  • Suggested edit: no repository source edit is justified by this cancelled check alone; the actionable next step is to rerun or wait for the current-head check that superseded it.
Failed check evidence for line-specific fixes

Failed GitHub Check Evidence

  • PR: #118
  • Head SHA: d7b9c4869bfd7f44a1e07179dc33bf02832d88b3
  • Repository: ContextualWisdomLab/.github

Line-specific repair contract

  • Treat the check logs and annotations below as diagnostic evidence, not as a complete review.

  • For each actionable failed check, inspect the local source or diff and identify the exact file line that must change.

  • OpenCode REQUEST_CHANGES findings must include path, line, root_cause, fix_direction, regression_test_direction, and suggested_diff.

  • Do not request changes with only a GitHub Actions URL or a generic check name.

  • When Strix logs contain multiple Vulnerability Report or Model ... Vulnerabilities ... sections, include every model-reported vulnerability in the review evidence and findings, including model name, title, severity, endpoint, and Code Locations/path:line evidence when present.

  • Create one OpenCode finding per Strix model vulnerability report; do not satisfy two model reports with one combined finding, even when titles or locations match.

Failed check: PR Review Merge Scheduler/scan-pr-queue

Check annotations

  • .github:1-1 [failure] Canceling since a higher priority waiting request for central-pr-review-merge-scheduler-ContextualWisdomLab/.github exists

The failed job log could not be collected with gh run view --log-failed.

Pull request overview

OpenCode model attempts did not emit a usable current-head control block, so the approval gate used deterministic current-head evidence instead of model prose.

Findings

No blocking findings.

Summary

  • Result: APPROVE
  • Reason: coverage-evidence passed, peer GitHub Checks completed without failures, mergeability was clean, and no unresolved human review threads remained.
  • Deterministic evidence: current-head changed-file evidence (.github/workflows/opencode-review.yml, scripts/ci/test_strix_quick_gate.sh); coverage-evidence result success; peer checks from statusCheckRollup excluding this OpenCode check.
  • Model outcomes: primary=failed, fallback=failed, second_fallback=failed, catalog_fallback=unknown.
  • Head SHA: 505d493453999f34ca6815a305f6702df5a737e8
  • Workflow run: 28371848064
  • Workflow attempt: 1

Deterministic fallback approval was used only after model-output instability and did not bypass coverage, failed-check, mergeability, or human-review gates.

HIGH OpenCode could not establish approval sufficiency

  • Problem: the model pool exhausted without a valid current-head review control block, so this changed line cannot be approved from deterministic check state alone.
  • Impact: PR-intent mismatches, missing files, robustness bugs, UX/DX regressions, and CodeGraph-backed flow changes could be missed.
  • Fix: rerun OpenCode after model availability recovers, or add the missing source/test/docs/generated verification evidence needed for a source-backed approval.
  • Verification: rerun the OpenCode Review workflow and confirm it emits APPROVE or source-backed REQUEST_CHANGES for this head SHA.

Closing during org-wide PR backlog reduction. This looks like a low-priority automated maintenance PR (test coverage only), while the repository has a large backlog of security, conflict, and required-workflow work. Reopen or recreate a fresh PR if this change is still needed.
