[codex] Make presets composition-first

Summary

• add an explicit intel-worker profile for the reviewed Intel worker layer
• make named presets expand through substrate + local-worker or substrate + intel-worker instead of hidden broad profile bases
• keep agentic and intel as lightweight compatibility routes with validator, CI, docs, tests, and decision-record coverage
