When you hit Insert, the "(OP)" string is missing from the game title. Acording to the OptiPatcher readme, that means OptiPatcher isn't loaded.
When you enable OptiScaler logging, you will see this line in the log
LoadAsiPlugins Checking Z:\home\deck\.local\share\Steam\steamapps\common\Expedition 33\Sandfall\Binaries\Win64\plugins\plugins for *.asi
Obviously that folder doesn't exist, so a possible solution is to create another "plugins" subfolder in the "plugins" folder and copy OptiPatcher into that. After that, the OptiScaler log will show OptiPatcher being loaded.
