More robust list of portals / destinations

* Have the option to provide a world for the "list" portal/destination subcommand. This allows you to view all the portals or destinations that are within the world provided. This makes portal management easier.
* Add a "near" subcommand. This will work similar to the list subcommand but the only argument will be a block radius to search for portals/destinations.
