Create form example page in UIDS

AIS is looking to create a 1.0 release of https://ais-components.its.uiowa.edu/ and would like feedback on their components.

Possible Slices

• Recreate https://www.maui.uiowa.edu/maui/pub/admissions/webinquiry/undergraduate.page (screenshot below) in UIDS Storybook combining https://github.com/uiowa/uiowa/blob/main/docroot/themes/custom/uids_base/scss/components/form/forms.scss and https://github.com/uiowa/uiowa/blob/main/docroot/themes/custom/uids_base/scss/webforms/webforms.scss to create form styles in UIDS.

Grooming conversation... 1st slice - May not need to recreate all form elements, just each different type of form element? Probably don't need to worry about the brand header since not directly consuming uids, focus on form elements. Single story in Storybook probably makes the most sense. Producing css that can be dropped into other application. Moving some form styles from uids_base into uids. - MEDIUM
