issues with VM scale.state consistency check on VM failures during scale up action #365
Description
When user requests toleration of VM failures, during Provisioning Orchestrator attempts to terminate the VMs and remove them from the list of the active ones of the Run. However, it doesn't or fails to set scale.state=removed. This breaks the consistency check later during Executing state on NEs.
