Add "Modern" design option

Add a selection to the plugin settings page that allows for a design selection. We need to add a modern (flat and square) design option.

• add a select option for "Standard" and "Modern"
• make "Modern" the new default setting
• if the new settings field is empty, treat it as "Standard" (support for existing installs)
• update the repo header image with a flat design
