chore: wip submitToolResult and tool approval flow

Checkpoint commit for continuing work on another machine.

Adds submitToolResult, switches tool approval away from in-memory Promise waiting, updates Vue/tool UI/demo wiring, and records the remaining awaiting-tool-results serialization work in TODO/AGENTS docs.

The full pause/resume requirement is not complete yet.
