When trying to add anything to the social history, medical history, ongoing concerns, reminders, they all give 500 errors

Needs to be migrated to Struts2 - core issue seems to be around null values from missing data elements (particularly blank ones / not set in a frm to pass back) and seeing we get the encType set correctly, as these are all the javascript / overlay window ones.
