[Bug]: Ocean eReferral: Attachments from other patients showing on referral

Description

When creating an Ocean eReferral, the attachment window may display documents that belong to a different patient.

Steps to Reproduce

Expected Result

Only documents belonging to the current patient should be attachable to their referral.

Actual Result

Documents from other patients can appear in the attachment list and be submitted with the referral.
