This is an additional enhancement to open-osp#197
Problem
When filing a lab on behalf of another provider, there are two different pop-ups. One that lets you choose which providers to file for, then another one to enter a comment for the acknowledgement. We have received feedback that this workflow involves too many popups.
Solution
Status Quo:
The "Automatically offer to file results on behalf of other providers when acknowledging HL7 lab results" option is defaulted to YES.
When you "acknowledge" a lab this opens first:

then after selecting who you want to file for and clicking yes/no you get this
Changes:

Update the user preferences for this feature so that the default is NO for both options

When the "Automatically offer to file results on behalf of other providers when acknowledging HL7 lab results" is set to YES, display a new modal that combines the two popups from the status quo behaviour

When the first option is set to NO, continue to display the old javascript comment modal.
