Provide a link to the GitHub
https://github.com/opendcs/rest_api/issues

Issues page from the web interface for "reporting issues". This will encourage users to report issues as they find them.

Bonus! If you can somehow via URI send what page the button was clicked from.

I.e. Report issue in header, if clicked from the "Portal/sources" would show it was reported from there. (Not sure if this is possible)
