I believe that is not really a PDF.js zoom bug.

On Safari iPadOS, pinch-to-zoom changes the browser visualViewport scale, not the PDF.js internal currentScale. That leaves PDF.js thinking the zoom never changed, so no scalechanged event is fired and the toolbar state becomes inconsistent.
In short: Safari takes over zoom, PDF.js loses control, UI breaks.

This is a platform behavior, not a logic error in PDF.js.
