[RFC] MCP Observability

We should define standard instrumentation for MCP servers. This includes metrics, traces, and logs with MCP-specific attributes y (e.g., mcp.tool.name, mcp.client.version, etc.), as well as a transport-agnostic way to propagate observability context between clients and servers.
