bats/assertions: Use lib/bats/helper-function

Closes #156. Also updates the documentation for both
`lib/bats/assertions` and `lib/bats/helper-function`, the latter of
which contains deep technical details about how Bats works and why the
`lib/bats/helper-function` mechanism is necessary for improved Bats test
performance.
