Add support for CSV header row

There doesn't appear to be a way to set a header row in a CSV output file. Please add an option to set a header row, ideally using the field names.

Is it possible to write only the header for the first line and not on each line?

The patch in #9 is out of date, but it looks like this would be much easier to add now since there's already a global lock around the file I/O.

I use the following trick to add header one time to the file

-generate a new file with the header
-populate it after with logstash
