[fix] Update changelog

Describe the bug

Looks like the current version is 5.0.0. However there's no info about breaking changes or anything like that. Could the changelog be updated?

@dlau published this 3 months ago and I am not sure why. The index.js file is the same as master minus the changes I merged in today. I assume is was a mistake?
