Here are the details:

• Library version: I used the latest version via npx @google/design.md, without a local package.json or local dependency.
• OS / shell: Windows, PowerShell
• Node version: v22.19.0
• npm version: 11.7.0
• Command: npx @google/design.md lint DESIGN.md

When I run the command in PowerShell, it produces no output. In some cases, Windows opens the DESIGN.md file instead of running the CLI.
The official command does not appear to require a local package.json or pre-installation. It is documented as runnable directly via:

npx @google/design.md lint DESIGN.md

My original reproduction used that direct npx form without a local package.json. The workaround that works on Windows PowerShell is:

npx -p @google/design.md designmd lint DESIGN.md

So the issue seems to be specific to invoking the package/default binary name design.md on Windows/PowerShell, not to whether the package is installed locally.
