Uniquely identify variables by variable index

Right now, variables are identified (for output purposes) by variable type and value reference. This should be changed, since uniqueness is only given if either variable name is included or the variable index -> the latter being the simplest to implement.
This is only an issue if several FMUs report variables with same value references and types.
