Epic: correctness fixes from package-by-package quality sweep

Package-by-package quality review (2026-07-03) surfaced 5 behavior/correctness issues (as opposed to pure refactors, which are deferred). These change behavior or hide failures from callers, so they go first. One issue = one PR = one changeset.

Children

• connectors: per-connector instructions is silently dropped by mountConnectors
• connectors: validation branches return success instead of MCP errors
• connectors: fetch rejections escape as raw TypeError, bypassing the typed error class
• mcp: resolveOrConstruct swallows resolver errors and builds an unwired instance
• ai-autopilot: DockerRunner symbols are unreachable from the package entry

Deferred to a later pass (not in this epic): untyped API-payload interfaces, shared cast/helper extractions (lazyModule, makeSafeEmitter, runner/spawn, asTool), and the test-coverage additions (tool-execution, web-handler, runner disk I/O).

• connectors: per-connector instructions is silently dropped by mountConnectors #146 -> fix(connectors): aggregate per-connector instructions into server metadata #152 (MERGED)
• connectors: validation branches return success instead of MCP errors #147 -> fix(connectors): return validation failures as MCP errors, not success results #154 (MERGED)
• connectors: fetch rejections escape as raw TypeError, bypassing the typed error class #148 -> fix(connectors): wrap fetch transport failures in the typed error class #155 (open, CI green)
• mcp: resolveOrConstruct swallows resolver errors and builds an unwired instance #149 -> fix(mcp): don't mask a resolver failure with an un-wired instance #156 (open, CI running)

Each is one PR + one changeset. Once #155 and #156 merge, this epic is done.
