About instruments

1. I've noticed that you index the instruments by channel rather than by track. It's probably the right thing to do, but you should then include a mapping from tracks to channels so that we can retrieve the instrument for each track.

2. I learned today that anything on channel 9 is a drum track (there may not be any program_change event).
