Additional source vocabularies requested

Is your feature request related to a problem? Please describe.
The CodeListGenerator has the feature "sourceVocabularies", which is very useful, but currently limited to selected source vocabularies e.g. SNOMED, READ, ICD-10CM.

Describe the solution you'd like
For phenotype development it would be great if ICPC (used in IPCI) could be added as sourceVocabulary, as IPCI data will be used in multiple studies and ICPC coding system is slightly different from those used in other DBs,

Describe alternatives you've considered
We could find codes at the stage of running cohort diagnostics in IPCI later

@ajoedicke as I think this refers to the shiny app, not the R package itself, so I've transferred it over to this repository. With the R package any vocabulary_id present in the concept table can be used. However, the vocabulary data behind this app doesn't have ICPC in it (and so doesn't appear as an option). I will look in to adding this.
