refactor: Rewrite ROADMAP to reflect actual release history and honest future plans

Replace 638-line outdated planning doc (still at v1.x era) with a
concise, accurate roadmap showing what has been released vs. planned.
Remove internal implementation notes and obsolete task checklists.
