Repeating items fields spacing improvement

Not a functional problem but an aesthetic improvement.

Describe the solution you'd like
The input boxes for repeating items seem to have inconsistent spacing. See image.

I think it would be nice to have a small gap between fields that is consistent. Or if no space is preferred, consistently apply that.

To reproduce the image:

1. Launch GPCCMP
2. Create new response
3. View "Carer and key contacts" section in the "Patient details" tab.

Here's another screenshot of spacing that doesn't look too good. There are 2 groups of "Carer and key contacts" that do not have an separation indication. Also the "Additional information" item at the bottom has not clear separation.
