"File Cannot be Saved"

Has anyone gotten the "File Cannot be Saved" when trying to running files through a queue to find metadata and artwork? Once it shows the error the file gets deleted. I checked the Messages log and see the below message:

18:48:15 The file “Macintosh HD” couldn’t be opened because URL type com-apple-unresolvable-file-reference-url isn’t supported.
18:48:16 The file could not be saved.

I've checked file and folder permissions, all are set correctly. One off files do not show the issue, only when running through a queue, and it's not all files at times.
