Harden PolicyFabric hook boundary
Add explicit policy-only decision boundary to the local PolicyFabric hook. Validation on 98160af: - Trust Surface: success - Control Plane Validation: success - ci: success This preserves the separation between policy decisions, runtime effects, authority mutation, state repair, and ledger writes.
