Install jquery ui vertical range slider into the 'view review' page

We are gonna use 1 comment form at the top, and let the user adjust the slider. The slider should go alongside the length of the file. We can use jquery ui to 'Modal Box' the comment form at the top so that it will appear no matter where the user is currently scrolled to. This will mean the 'comment' button has to be accessible at any height of the page somehow.
